TO PLANHOLDERS COMMUNITY DEVELOPMENT BLOCK GRANT PROJECT WEST
AVENUE IMPROVEMENTS ADDENDUM NO.2 I. Changes to the Federal Requirements and Wage Decision: A. Please be advised that the Federal Prevailing Wage Rates were modified by General Decision Number CA020009, Modification Number 6, dated May 17, 2002. Reference is made to the penultimate paragraph, "Pursuant to Section 1773…" on page SP-2 and Section 5-1.02 of the contract specifications. II. Changes to the Contract Specifications: A. Page SP-1, the last sentence of the first paragraph is revised to read: "Said bids will be referred to the Sonoma County Board of Supervisors for consideration at their next regularly scheduled meeting, on or before August 6, 2002." B. Page SP-40, add the following paragraph to Section 10-1.04, "OBSTRUCTIONS": The Contractor shall verify the exact depth and location of the existing underground gas main within the project limits, the underground gas line in the vicinity of Station 5+59 Right, and the electrical facility in the vicinity of Station 7+51 Right by potholing. The County is awaiting verification from PG&E of the approximate location of the underground gas main. The Contractor shall pothole at least five (5) days prior to the construction of the work. The Contractor shall notify the owner of the utility at least 48 working hours in advance of the potholing operations. Full compensation for potholing shall be considered as included in the contract prices paid for the various contract items of work and no separate payment will be made therefore. C. Page SP-52, add the following to Section 10-1.14I, "RELOCATE FENCE": Existing gates shown on the plans to be relocated will be removed and reset in accordance with the details shown on the plans or as directed by the Engineer. Existing gates to be relocated shall be measured by each. The contract price paid per each for relocate gate shall include full compensation for furnishing all labor, materials, and equipment and for doing all work involved in relocating gates as needed and no additional compensation will be allowed therefore. Retitle Section 10-1.14I, "RELOCATE FENCE AND GATES." D. Page SP-81, add the following to Section 10-1.31, "FENCES": Temporary fencing shall be installed as per the direction of the Engineer. The contract price paid per meter of temporary fencing shall include full compensation for furnishing all labor, materials, and equipment and for doing all work involved in installing and removing temporary fencing as needed and no additional compensation will be allowed therefore. III. Changes to the Plans: A. Sheet 6, Plan and Profile There is an existing underground electrical line in the vicinity of Station 7+51.5. There is an existing water meter and service line in the vicinity of Station 7+58 Left. The existing barricade at the end of the existing sidewalk in the vicinity of Station 8+48 Right shall be removed and disposed. B. Sheet 8, Details Driveway Detail: All driveway conforms beyond the back of the concrete sidewalk shall be 76.2 millimeters (3 inches) of full depth asphalt concrete. C. Sheet 11, Quantity Summaries The quantity of Type I Fence from Station 7+76.33 to Station 8+26.08 Right changes to 36.38 meters. The quantity of Type I Fence from Station 8+26.08 to Station 8+48.94 Right changes to 25.78 meters. The total quantity of Type I Fence changes to 71.77 meters. For the fencing quantity summary, add to the "Remarks" column that the open wood rail fencing from Station 6+88.58 to Station 7+55.92 Right and the picket wood fencing from Station 7+ 55.92 to 7+72.18 Right shall be painted white. IV. Changes to the Special Provisions, Engineer’s Estimate and Bidder's Book: A. Add new Item No. 1, "Temporary Chain Link Fence," (Item Code No. 071322), the quantity is 160.36 meters. B. Item No. 19 (now Item No. 20), "Cold Plane Asphalt Concrete Pavement," the quantity is changed to 675.4. C. Item No. 43 (now Item No. 44), "Jute Mesh (Ditch Lining)," the quantity is changed to 252.0. D. Item No. 44 (now Item No. 45), "Minor Concrete (Sidewalks, Driveways)," the quantity is changed to 593.0. E. Item No. 49 (now Item No. 50), "Type I Fence," the quantity is changed to 71.77. F. Item No. 56 (now Item No. 57), "Thermoplastic Pavement Markings," the quantity is changed to 78.05. Please replace pages SP-4 and SP-5, BB-3, BB-4, BB-23, BB-24, and BB-25 with the attached revised pages (SP-4, SP-5, BB-3, BB-4, BB-23, BB-24, and BB-25) to reflect the changes. Please fill out the Addendum Acknowledgment sheet (page BB-5 of the Bidder's Book) verifying your receipt of this Addendum (Addendum No. 2). EDWARD
