OVERVIEW (What is MSSP?)
MSSP is a care management service for seniors. The MSSP care management team is made up of a social worker and a public health nurse. MSSP care managers work with the client, family members and others to find and coordinate services in the client’s home.

Who is Eligible?
MSSP clients are seniors age 65 and older who are eligible for Medi-Cal and in frail health or at risk of going to a nursing home. Clients must have an ongoing need for care management and be willing to accept MSSP services. Most all MSSP clients also receive In-Home Supportive Services. MSSP clients must live in Sonoma County.

What kind of help is available?
The client and care manager work together to determine what services are needed in order to stay at home as safely and independently as possible. The care manager may refer the client for additional services or MSSP may purchase a limited amount of additional services for clients.

The care manager and the nurse interview the client at home and complete a comprehensive psychosocial and health assessment. A care plan is developed with the client and family. Services are arranged or purchased to assist the client to remain independent. The care manager calls regularly and also visits each client to monitor and re-evaluate the client’s needs.
