Seniors Chronic Disease, including cancer, heart disease, stroke, chronic lung disease and diabetes, accounts for the majority of deaths in Sonoma County. Many chronic diseases are considered preventable and are linked to lifestyle behaviors such as smoking, alcohol abuse, obesity and lack of exercise.

Prevention efforts currently underway include the Northcoast 5 a Day—Power Play! Program, the Community Activity and Nutrition Coalition, the Local Incentive Award Program, Healthy Eating Activing Living Community Health Initiative, the Tobacco Use Prevention Program and Alcohol and Other Drug Prevention Program activities. Program staff members are involved in several collaborative projects aimed at increasing the activity level and fruit and vegetable consumption among Sonoma County residents.
