Nominations for Public Health Champions

National Public Health Week will be celebrated April 7-13 this year. This national celebration provides us with the occasion to recognize the contributions of public health to the nation's well-being as well as focus public attention on major health issues in our communities. Our local celebration will include spotlighting local Public Health Champions - individuals whom we honor for their outstanding accomplishments in promoting the health of Sonoma County.

Nomination Process

We invite you to submit your nominations (pdf) for Sonoma County Public Health Champions. Nominees should be those who have demonstrated leadership, innovation, and creativity in addressing public health issues. Their contributions should be either voluntary or beyond their normal scope of work. Individuals, teams, or partnerships may be nominated. (Department of Health Services employees may be nominated. However, their contribution must be outside of or beyond their DHS scope of work.) Please designate nominees for recognition in any one of the following categories:

Selection Process

A local committee of community and government representatives selects the Public Health Champions from the nominees. Selection is based on the criteria stated on the nomination form (pdf). Nominees chosen as Public Health Champions will be informed of their selection by March 25, 2008.
