Teen Parent Connections (TPC) is a program for pregnant or parenting teenagers age 18 years or younger. As part of Sonoma County Public Health's home visiting programs, TPC provides long-term comprehensive case management services to improve the young parent's health and the health of their infant, promote nurturing parenting skills, maximize their educational potential and advance self-sufficiency and healthy lifestyle choices.
Clients are connected to medical providers, schools, WIC, counseling, legal and other supportive services. Referrals are accepted from schools, health care providers, county- and community-based agencies. Self-referrals are also accepted.
At the Department of Health Services (DHS), we believe strongly that taking care of the children of our community is vital to its health and well-being. And when those children find themselves soon-to-be parents, we are there to help them and their babies get off to a good start and stay on the path to a bright future.
Our goals include:
- Reduce the incidence of low birth weight babies.
- Maximize educational potential.
- Reduce the incidence of repeat unplanned pregnancy.
- Foster positive parenting skills in young parents.
It is our vision that these teens will form healthy relationships, and achieve for themselves and their children safe and secure futures.
