Project ReConnect Tool Kit

Program Operations

Project ReConnect was activated with ReConnect Advocates (RAs) trained, hired and in the field operating projects that range from:

• Working directly with consumers.
• Helping consumers re-engage in their lives and communities.
• Supporting employers in welcoming people living with HIV into their workplaces.
• Assisting service providers who are transitioning into delivering HIV services.
• Meeting with community groups to spread the word about the Project ReConnect philosophy.

Sonoma County Department of Health Services staff and a Project ReConnect coordinator manage their projects. Sample management tools include:
