Families First: For Health Providers

The Families First home visiting program offers "universal" home visits free for all first-time parents who live in Sonoma County. Eligible parents include first-time mothers, first-time fathers, parents who adopt, and those who are parenting an infant for the first time.
The visits follow an outreach model and are not medical or case management. If there are medical concerns, parents are referred back to their primary care provider.
Families First provides new mothers and fathers with three visits during the baby's first year.
All services are available in English and Spanish.
There are three ways for families to receive a visit:
- Parents can sign a form at the hospital or birth center when the baby is born.
- Parents can self-refer by calling us at 707-565-4507.
- Parents can use our online form to request a visit.
