Request for Proposal #CX3-A111411: Communities of Excellence (CX3) Program Assessment Contractor

The Sonoma County Department of Health Services (DHS) Public Health Division is pleased to announce the release of a Request for Proposal (RFP) for an agency/organization/individual to conduct Communities of Excellence in Nutrition, Physical Activity and Obesity Prevention (CX3) food environment assessments in the following four (4) neighborhoods: Boyes Hot Springs (Sonoma Valley), Roseland (Santa Rosa), Kawana Springs/South Park (Santa Rosa), and downtown Santa Rosa.
