The Department of Health Services Public Health Division intends to award contract(s) to:

The Department wishes to thank all of the organizations that submitted a response to this Request for Qualifications.

Organizations that were not selected for an award have the right to file an appeal if they believe that a significant material error occurred in the evaluation of proposals and/or the selection process. Appeals must be submitted in writing by October 7, 2011. The Director will issue a response to all appeals within ten (10) working days of receipt of the appeal. Please submit appeals to:
