The new recruit will be responsible for filling out the following forms. After Sonoma County Fire's Administration Division has received all items, the Administration Division will notify Kaiser that the new recruit will be calling to schedule an appointment. The new recruit must bring the completed Medical Questionnaires, to the medical evaluation.
