Forms: Underground Storage Tanks (UST)

If you own or operate underground fuel storage tanks in California, you are responsible for ensuring that the following forms are filed with your CUPA. The following are PDF versions of UST forms. You will be able to read them and fill them out on-line using Adobe Reader, which you can download for free if you don't already have it. However, you won't be able to save the completed documents to your computer unless you have Adobe Acrobat, which must be purchased.
