How and where do I get an application for Child Support Services?
Answer
First decide which nonwelfare application packet below fits your circumstances
and then download all of the associated forms for that selection. You
may also request an application by calling us, or you may pick up an application
at our office. If you are applying for, or are now receiving public assistance
(TANF) you will receive child support services application information through the Human
Services Department.
- I am a mother. My child(ren) reside with me. There is no court order requiring the other parent to provide monthly child support payments and/or health insurance for my child(ren).
- I am a mother. My child(ren) reside with me. There is a court order requiring the other parent to provide monthly child support payments and/or health insurance for my child(ren).
- I am a father. My child(ren) reside with me. There is no order for the other parent to provide monthly child support and/or health insurance for my child(ren).
- I am a father. My child(ren) reside with me. There is a court order requiring the other parent to provide monthly child support payments and/or health insurance for my child(ren).
- I am the guardian for child(ren) who reside with me. There is no court order requiring the child(ren)'s parents to provide monthly child support payments and/or health insurance for the child(ren).
- I am the guardian for child(ren) who reside with me. There is a court order requiring the child(ren)'s parents to provide monthly child support payments and/or health insurance for the child(ren).
