The Assessor forms can now be filled out on your computer! Filling forms out on your computer helps to expedite processing, increases accuracy and provides a clear copy for your records. Simply open the document in Adobe Reader, tab through the fields (there are tool tips!) as you type your answers. Print two copies of the form (one for your records), sign one copy and send it off to the address on the form - it is that simple!
All of the forms listed below are available for download in PDF format; some are combined into packets as PDF Portfolios. Click on the name of a form or packet to download it. You can read additional information about some of the forms by clicking on links next to them that say "Additional Information".
